Forms and Applications

As soon as you decide to apply for a grant, you must fill out a PIAF (Proposed Internal Approval Form) and send it to the Office of Research and Sponsored Projects. You can link to an online version of the PIAF from the ORSP Web site.

ORSP's Web site also offers information on PSU policies, compliance issues, internal funding, and research funding sources.

Agency or sponsor pages require specific names, addresses, identification numbers, codes, or other information, many of which are listed on ORSP's Required Information for Sponsors page

NIH Public Health Service Grant application form 398

To apply for NSF grants you must be a FastLane member and submit your proposal electronically. PSU is a registered FastLane member, ID# 0032169000. You will also need to register yourself individually, but must notify Barbara Sestak before you register so you can add you as a new user. Contact ORSP with the following information: First Name, Last Name, Middle Initial, Suffic (e.g. Jr. Sr, III), Social Security Number, Department, Degree Type and Degree Year, Street Address, City, State, Zip, Email Address, Business Phone, Fax Number.

A complete list of instructions for becoming a FastLane member and preparing your proposal can be found at the NSF FastLane Instructions for Preparing and Submitting a Proposal page. Other helpful pages from FastLane include: 10 Steps to Prepare Proposal. Because this process may be unfamiliar, it is a good idea to go to the FastLane site ahead of time and familiarize yourself with the process.

Many federal grants require that you use forms SF 424, 424a, or 424b. Download those forms here.
